Health Guarantee
I, Jennifer Brady, state that I have sold a _____________________________________to ______________________________________on ____________ for the sum of __________________.
I also state that this dog is in good general health, UTD on vaccines and free from life threatening genetic defects. This guarantee does not cover such parasites as giardia, coccidiosis, fleas, upper respiratory infection or ear mites. In order for this guarantee to become valid, the dog must be seen by a licensed vet within 48 hours from the date of this document. If the dog is not taken to a vet within 48 hours from the date on this document, this guarantee is void. I am not responsible for any injury or disease acquired after the dog leaves my care. Nor am I responsible for any extra costs incurred from the airline (if flown) should the dog have a lay over due to inclement weather or other unforeseen reason. If the dog is found to have a “life threatening “genetic disorder by the vet, the dog will be replaced with a dog of my choosing of equivalent quality from the first available litter at my option. There will be no refund of the purchase price. Absence or retention of testicles is not guaranteed nor is the fertility of either male or females guaranteed. I do not guarantee size or personality. Allergies are not covered in this guarantee.
I have enclosed the dog’s health record and registration papers. The new owner must complete and submit the registration papers at their option and expense. This dog is sold as pet quality only. Show or breeding quality is not implied nor guaranteed. I do not refund purchase price because your landlord says you can’t have a dog, please check with your landlord first.
If you are unable to provide this dog a good home and adequate care please call me so that I may either choose to take the dog back, at no cost to me, or assist you in placing it in an appropriate home. Please don’t take the dog to the animal shelter or dog pound. It is your responsibility to provide this dog with ALL further health care and vaccinations and give it adequate care in your home without cost to me. By signing this form you state that you have read it carefully, understand its contents and agree to this contract.

Willows End Kennels reserves the right to refuse to sell to any prospective buyer without explanation. This decision is final and totally at my discretion. You will receive a refund of your deposit if the pending sale is terminated by me. The welfare of my pups in their future home is my priority, and if I feel that there may be an issue with regards to the pup’s mental or physical wellbeing, I will terminate a sale.
I will make every effort to keep you updated on your pup’s progress if you will request info via email. However, it is my first priority to care for my family and my dogs. Therefore emailing pictures and updates and such is not guaranteed and will be done when I can fit it into my hectic schedule. I know you are excited about your new family member but you will want him/her to receive the best care I can provide. Proper puppy and mommy care is very time consuming and labor intensive at all hours of the day and night. Please be patient with me if I don’t respond to your emails until the next day or so.
We are required by state law to charge 6% Kentucky sales tax. A 100 dollar non-refundable deposit will hold your pup until it is old enough to leave mom and me, usually eight weeks, but may be older if it is a teacup sized pup. Pups sold at pet prices will be sold without papers or with limited registration papers at my discretion.
